Information we need to schedule your appointment

Hey!

Thanks so much for using our NEW self scheduler! If you are a new patient we'll need some additional info from you before we can confirm your appointment to ensure we can give the best possible service possible. You may add it into the additional notes section or email it :)

Case Type: we will need to know whether you plan going through insurance, paying out of pocket, or even workers comp or auto claims. This allows us to verify your coverage prior to your treatment and to eliminate any unwanted bills following your treatment. If do you plan on going through insurance, please include the carrier name, plan type (PPO, HMO, POS or EPO), Identification number and your date of birth.

Credit Card Number: A valid credit card will be required to book your appointment but it will not be charged, it is only to reserve your appointment time. For any reason you need to cancel or reschedule please do so at least 24 hours in advance. Same day cancellations will result in a 50% charge of service, and no shows will be charged 100% charge of service.

Late Appointments: If you are running more than 10 minutes late, We will do everything possible to accommodate and give you the full service. However, if it impedes upon the following appointment we may be only able to do a partial treatment or require you to reschedule.

Please note that if I do not have this information, your appointment is not confirmed. It will be put into the system tentatively, but if I do not get that info 24 hours after it has been scheduled it will be removed. Thank you for understanding
