If you are a new patient we'll need some additional info from you before we can confirm your appointment to ensure we can give the best possible service possible. You may add it into the additional notes section or email it :) If you plan on using health insurance, please include the carrier, member ID, and date of birth. If you do not wish to use insurance then please add in the note section that you will be a cash patient. Please note that if I do not have this information, your appointment is not confirmed. It will be put into the system tentatively, but if I do not get that info 24 hours after it has been scheduled it will be removed. Thank you for understanding
